• Public Service Reporting Form
  Use this form to report public service presentations (Police / Fire / EMT / Hero / Lifesaving) to the National Office.
  You must download the document to your computer, enter the data, and save the document.
  Then, press the Send button to forward the document to the proper National SAR Committee.
  Failure to follow this exact procedure will result in National not receiving your report.
